Under Georgia law, mental health records are privileged and confidential. No one outside the Stamps Health Services professional staff may have access to your records and no one outside Stamps Health Services may be given information without your consent. Exceptions to confidentiality may occur only under certain circumstances. These exceptions include serious immediate threat to your life or welfare or to the life or welfare of another person or the university community.

You may make a request in person in the Psychiatry office at the health center.   Please note that there is a charge for production of medical records. Please call (404) 894-2585 for more information.

If you are unable to make a request in person at Health Services, you may download the Psychiatry Release of Protected Health Information form* by following the link.  Please complete the form in its entirety with an appropriate signature and fax or mail the form to Health Services - ATTN: Psychiatry Medical Records.

In addition, you may make your request in writing.  Be sure to include your nine digit GT Id number, Date of Birth, address to send your records, years attended, daytime phone number and your signature.
